* Must be completed and submitted when a change of Chapter Director or Treasurer occurs or on an annual basis.
* Outgoing Chapter Directors completed report from ending date of last annual report to change date. The report will be submitted as a “This is a change of Officer Financial Report”. The change report can also be completed in combination with the Annual report.
* Contact the District Treasurer for assistance completing the Annual or Change of Director/Treasurer Financial Report.
* The following forms will be completed for ALL new Chapter Directors, Assistant Chapter Directors, Rider Coordinators, Membership Enhancement Coordinators and Treasurers.
